PROF KALBASSI COLORECTAL CANCER IN YOUNG WOMEN SCI BUZZ Prof Reza Kalbassi Consultant in Colorectal Surgery Keen to raise awareness about colorectal cancer presenting late in younger people Professor Reza Kalbassi shares what the most common things to look out for are and why it s so important not to dismiss even the most minor changes to your health Oftentimes patients dismiss their symptoms or their presentation is delayed but there are several signs to look out for Symptoms that warrant urgent investigation include rectal bleeding persisting for longer than six weeks a change in bowel habit to constipation or diarrhoea looser stool or increased frequency persisting for more than six weeks PROF WESTRUP PRESCRIPTION EXERCISE Exercise may be better for your health than many medications by Prof Jennifer Westrup Consultant Medical Oncologist The health benefits of exercise are well known in cardiovascular disease respiratory conditions and diabetes Exercise improves your mood But did you know that exercise is linked to a reduction of developing cancer We know from large studies that people who exercise develop fewer cancers In fact exercise has been linked with a lower risk of 13 different cancers including cancers of the breast colon endometrium oesophagus kidney liver stomach head and neck rectum bladder lung and certain blood cancers In the Cancer Prevention Study 3 The American Cancer Society and the National Cancer Institute reported that moderate to vigorous exercise reduces cancer risk Moderate exercise is 150 minutes per week and can be achieved by a brisk 30 minutes walk on your lunch hour during the working week Vigorous exercise goals are 75 minutes per week Young patients tend to present later and often ignore their symptoms for a period of time resulting in diagnosis of a more advanced disease Prof Kalbassi stresses the importance of early intervention in the treatment of this cancer Exercise also has benefits for people already diagnosed with cancer Side effects of cancer treatment like fatigue pain and nausea can be improved with regular exercise The Dutch PACT study proved that patients could not only exercise successfully while on cancer treatment but that they were also more likely to continue exercising in following years which had additional benefits on health and well being Exercising after a cancer diagnosis has been proven to reduce the risk of recurrent cancer INDUSTRY particularly in breast endometrial prostate and colon cancer If you are concerned about any health issues don t delay Visit your GP and get it checked Physical activity lowers your risk of developing cancer and having a recurrent cancer It improves mood and cancer treatment related symptoms Your prescription Exercise 30 minutes a day Increase intensity at regular intervals Start now Beacon Hospital News Issue 1 April 2023 BEACON HOSPITAL 4

5 Use but don t overuse Technology 7 Relaxation Watches trackers and phones can all support your goals of more exercise relaxation sleep and stress monitoring These are very worthwhile Try to find small ways to relax each day be it Tai Chi But on the other hand using the same gadgets all day and night are not good for your health and in turn your heart They reduce brain function affect sleep and increase mental health issues especially from excessive online social media use Limit your daily phone use Yoga Meditation Mindfulness Reading Singing having a pet writing or getting a massage Some of these activities lower cholesterol levels and improve blood pressure control More broadly having an optimistic grateful positive mental attitude improves heart health and longevity 8 Meet and Socialise Socialisation is the most important health factor for 6 Stress Recovery Everyone feels the effects of stress some more than others and covid has not helped Chronic stress is independently associated with 30 increased risk of heart disease irrespective of presence of smoking blood pressure diabetes obesity and inactivity Dealing with stress requires a multidisciplinary approach looking at reasons for stress adopting simple behaviours to lower stress levels building resilience by improving physical health measures such as exercise eating sleeping reducing alcohol intake stopping smoking and it may require seeing a counsellor psychiatrist and going on medication There are many small behaviours develop a gratitude habit learn to say No breathing exercises that you can undertake that help lower stress levels while you build up resilience and deal with reasons for stress Beacon Hospital News Issue 1 April 2023 living a long healthy life It s important to find time to meet with family friends and work colleagues If you re struggling think of joining a club or starting a team sport and a new season is often a good time to do this Covid highlighted cocooning and effect of isolation and loneliness The latter increases risk of heart disease by 30 9 Reduce Alcohol Intake Reduce or stop drinking if you have been drinking excessively Europe recommends 14 units 1 unit small glass of wine half pint of beer per week for men and 10 units for women not all in one night Of course less is better 10 Have a Medical Examination Never miss out on getting your cancer smears and UP Rather ENTERthan scans or getting your bloodsSIGN done waiting for something to happen take a proactive approach to your health BEACON HOSPITAL 7
PHYSIOTHERAPY MAXIMISING PERFORMANCE FOR THE GAA CHAMPIONSHIP SEASON Ian Byrne Chartered Physiotherapist Beacon Hospital With the new GAA season format teams will now be entering a more intense training block in preparation for an August Club Championship This will be an exciting time for players and supporters with Championship games being played in fine weather on hard ground and fast pitches Are there any better conditions for Championship Football Hurling or Camogie There will be some unfortunate souls who fall victim to injury in this period and there is nothing worse than picking up a potentially preventable soft tissue injury niggle and chasing your fitness for the rest of the Championship With that in mind here are some pointers on staying injury free and maximising performance for the white heat of Championship Periodisation and Planning Periodisation is the deliberate planning or structuring of training in order to optimise or peak at the important times Sound periodisation is a key part of staying injury free If you re preparing for an intense period Beacon Hospital News Issue 1 April 2023 of group games then replicating the demands of these games is important If your training is containing low intensity high volume running for example are you adequately preparing your body And will your body cope with the demand of 60 70 minutes of high intensity running Similarly if gym based resistance training is still part of your training week it should likely look quite different than your offseason gym routine Do you need to lift heavy for numerous reps or will sharp power based plyometric training allow you to be more explosive in tackles or evading the opposition Note that this power based training can be more demanding and should be considered in the context of your training week Rest and Recovery Having spoken about the planning of training potentially more important is the planning of your recovery As we reach the group stages early phase of Championship the heavy work should be done and the focus will be on maintaining performance for a run of consecutive games Championship intensity may take your body to a place it hasn t been to in a while and you need to ensure you are sufficiently recovered between games Evidence suggests that athletes who sleep less than 8 hours per night are up to twice as likely to be injured Consider sleep hygiene tips like BEACON HOSPITAL 8

UROLOGY ENLARGED PROSTATE SCI BUZZMr Michel deWildt Consultant Urologist Benign enlargement of the prostate which encircles the peeing tube and can lead to restriction of urinary flow out of the bladder can cause bothersome urinary symptoms that affect quality of life by interfering with normal daily activities and sleep patterns Mr deWildt confirms treatment usually begins with medication but as this medication becomes less effective a more definitive solution is desired by patients Traditionally the preferred surgical treatment was TURP TransUrethral Resection of the Prostate however a recent novel surgical approach Holmium Laser Enucleation of the Prostate known as HoLEP has more results permanent compared long term to TURP There is also a significantly shorter recovery time and minimal side effects after the HoLEP surgery When medication for urinary symptoms are not that effective anymore this should be discussed with your GP who can refer you to a urologist to perform additional tests to see if surgical intervention like HoLEP is necessary Beacon Hospital News Issue 1 April 2023 BEACON FOR KIDS FOOD ALLERGIES IN CHILDREN Food allergy occurs in approximately 5 of Irish Children by Prof David Coghlan Consultant Paediatrician Allergic responses can be divided into IgE and non IgE mediated reactions IgE mediated reactions are characterized by urticaria hives angioedema swellings vomiting and wheeze following food exposure Anaphylaxis occurs when there s an extreme response involving not just the skin but also the child s respiratory system cough stridor wheeze and cardiovascular system involvement lethargy faint collapse low blood pressure These reactions usually occur within minutes of food consumption but may take up to two hours in some circumstances the most common food triggers in children include milk eggs peanuts tree nuts and fish Food allergy is associated with other allergic conditions including eczema and asthma The evaluation of these children involves a team approach with the general practitioner paediatrician nurse and dietician A detailed history is the most important element in identifying food triggers Following this a child may have skin prick testing or specific IgE testing performed The advantage of skin testing is that it s an accessible low risk procedure in trained hands with the child and family getting their diagnosis confirmed on the day of clinic A cohort of children will require food challenges that should only be performed in paediatric settings with access to emergency medication and appropriately trained staff Many children will require adrenaline autoinjectors as part of an acute action plan Any food can potentially trigger an anaphylaxis reaction Children with associated asthma are at a higher risk of severe reactions and need their acute asthma plan reviewed Children should have an annual medical review of their general health allergy status and have their anaphylaxis plan revised and adrenaline autoinjectors prescribed as necessary BEACON HOSPITAL 10
